Service Description

We will meet either in person or online to complete your needs assessment. This will bring clarity to what services are working, what aren't and why, and what services are required. This support item strengthens a participant’s ability to design and then build their supports with an emphasis on linking the broader systems of support across a complex service delivery environment. Coordination of Supports focuses on supporting participants to direct their lives, not just their services, and is focussed on assisting participants to build and maintain a resilient network of formal and informal supports. This involves working together with the participant to understand the funding, identify what participants expect from services, and how participants want this designed. Coordination of Supports also includes coaching participants, and working with participants to develop capacity and resilience in their network.

Cancellation Policy

Cancellations within 48hours will incur a full fee. Up to 2 re-schedules incur no fee. A third re-schedule may or may not occur, depending on your reasoning. Please be courteous and notify us by phone or email if you are unable to attend an appointment at least 2 working days (Monday to Friday) prior to your scheduled appointment. We will then be able to allocate this time to someone on our waiting list. For example, for a Tuesday appointment, please notify us by the Thursday before.
